Community Based Specialist

Summary:
Responsible for the overall quality assurance and operations of all Community/Home Support management?services in the State. This position will be responsible for compliance with all Community/Home Support Programs, retention and compliance with requirements through MaineCare/Medicaid and funding source policies and procedures.

Supervisory Responsibilities:
This position is responsible for the oversight of Direct Support Professionals, if applicable.
?
Essential Duties and Responsibilities:
1.??????Serve as the regional liaison to Department of Health and Human Services, Agency for Persons with Disabilities and other funding sources as assigned.?
2.??????Responsible to review and ensure submission of all required documents to be uploaded into SharePoint as required in compliance with State Regulations.
?
3.??????Responsible for ensuring all Goals are up to date in Therap according to the Individuals Person Center Plan.
?
4.??????Responsible for ensuring all Individual Contact Information is up to date in Therap.
5.??????Responsible for ensuring all programs are in compliance with all state and funding source regulations, policies and procedures.
6.??????Attend all Individual meetings scheduled with funding sources.
?
7.??????Attend all scheduled PQI meetings.
8.??????Ensure site visits are conducted for all Community/Home Support programs based on funding source requirements which are one time per month in person and bi monthly phone visits. Ensure that these visits are documented in Therap within 24 hours of the visit.

2.???????Responsible for maintaining staff compliance at all times in regards to documentation, maintaining up to date files, trainings and receive ongoing support and supervision.

??????????Ensure all service standards are met for all services to include but not limited to timeliness of assessments and treatment/service plans; up to date releases etc.

??????????Ensure all documentation (billable) is completed with 24 hours of services being rendered.

??????????Work closely with Maine?s PCC and Direct Supervisor to ensure that documentation is being completed and submitted in a timely matter. Responsible to review and ensure submission of all required documents to be entered in Therap as required to comply with licensing.
10.??Consult with Guardians, Case Managers, Providers, and Individuals on a regular basis.
11.??Ensure all incident reports are submitted within 24 hours to funding source and all team members are notified.
12.??Attend all team and supervision meetings.
13.??Provide on call coverage as backup if needed.
1.???????Other duties as assigned.
15.??Phone calls and emails are returned within 24 hours.
1.???????Compliance with the agency policies and procedures is mandatory.
1.???????Represent the agency in a professional and positive manner.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
