Unity Living Center focuses on providing treatment and rehabilitative care for patients with medically-complex needs, dementia and behavioral challenges; we offer a community-based approach to both long-term care and post-acute care.
AWARDS & RECOGNITIONoFour-star rating from the Centers for Medicare and Medicaid Services (2020)ATTRIBUTES Nursing Degree required, BSN preferred Minimum three (3) years acute/continuing care clinical experience MDS proficiency preferred Strong leadership and communication skills RESPONSIBILITIES Daily Management.
Ensure safe and appropriate staffing levels and skills during all hours of operation; maintain ongoing communication with staff regarding performance; develop and assess department goals and objectives; act as a resource to department, staff, patients and other department staff and the community Leadership.
Act as a liaison and facilitate the flow of information; assure completion of incident reports and patient complaints with appropriate follow-up; support and communicate standards of care, management philosophy and objectives; provide input regarding departmental budget for staffing supplies Patient Care.
Develop the resident care plan and ensure implementation through the interdisciplinary team; develop, implement and evaluate in conjunction with Senior Director of a department Quality Improvement Plan EDUCATION:
LICENSES / CERTIFICATIONS:
BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
